Before attending your medical and to ensure we can process the medical smoothly we would request you bring the following items with you:
A completed form
A valid passport for identification
Last medical certificate
Signed medical consent form
Glasses/ contact lenses if worn with an up to date spectacle prescription. Contact lens wearers should attend wearing glasses.
A chaperone if you wish to have one during the clinical checks
A means to pay for the assessment on the day
Ideally we’d also like to review your health record which can be requested as a ‘Patient Summary’ from your GP
